I declare that all the particulars given and informed in this application are true and correct. I did not wilfully withhold or suppress any other relevant information.

I understand that:

1. Submission of any false information will render my application void and I will be required to refund Jamiyah Singapore the full value of the scholarship received;

2. The scholarship awarded will be paid directly to the applicant via bank transfer;

3. I am required to fulfil 50 hours of volunteer work with Jamiyah Singapore (includes all entities within Jamiyah Singapore) per year during my course of study.

4. The Scholarship Committee reserves the right to allot my scholarship application (subject to eligibility) and the decision of the committee is final;

5. The Scholarship Committee reserves the right not to disclose its reasons for approving or disapproving an application;

6. The Scholarship Committee reserves the right to withdraw or suspend at any time and scholarship in full or partially if conditions are not met;

7. The documents submitted with this application will not be returned;

By submitting my personal data, I consent to the Scholarship Committee and Jamiyah Singapore collecting, using and disclosing my personal data for communication, administrative, academic purposes, research and statistical reporting.
